Quality Improvement audit and learning project

Thrombosis UK invites primary care practices and primary care networks (PCNs), to take part in a Quality Improvement learning through audit programme.

Focusing on the pathway to VTE diagnosis ‘Detect VTE’ QI project is open to applications from UK primary care providers. Registration of interest to be part of this project must be received by 31st December 2023 with meetings leading to initiation of the QI learning audit in January 2024.

Objectives:

  1. To identify common patterns in pathways leading up to diagnosis of a VTE by patients and clinicians.
  2. Identify those which support early suspicion and diagnosis of VTE as well as those which may challenge or impede diagnosis.
  3. Identify solutions that will add value to clinical decision making and have a positive impact for patients experiencing a VTE.

Benefits to the practices involved:

Participation in this opportunity if being offered FREE of charge. Each participating practise will receive a full report sharing data and learning gained during the QI programme undertaken in their practice which can be submitted as part of their GP and CQC revalidation work.

  • The QI learning through audit is fully funded by Thrombosis UK.
  • The work will be supported by a clinical solutions team and with over two decades of experience working with the NHS.

  • No costs and minimum work burden for the practice staff.

  • As an outcome of this programme, if a clinical solutions tool to aid early identification of VTE is developed, participating practices will be invited to trial this, free of charge. The end goal being to make this available free of charge to NHS providers.

Benefit to patients

Venous thromboembolic events have a life-long impact. Related complications are frequent and include pulmonary hyper-tension, post thrombotic syndrome and increased risk thrombotic risk.

It is expected that learning as a result of this work will:

· Improved prevention of catastrophic, sudden, premature, death which is almost wholly preventable.

· Expected improvement in patient outcomes and experience.

Eligibility:

  • Open to NHS primary care settings and PCNs.
  • No minimum practice population size is required.
  • Practices from a diverse range of settings including: geographical location, socio-economic population, operational systems and invited to apply to participate.
